We are currently looking for an Information and Support Worker to work for a leading support and research charity. The role is a permanent home based position covering Haringey, Hackney, Tower Hamlets, Islington and the City of London.
The main purpose of the role is to ensure the one to one provision of relevant information, advice and advocacy to sufferers of neurological conditions, their families and carers within a defined geographical area. This will include signposting within the charity to relevant care pathways; appropriate healthy and social care networks and other voluntary organisations and specialist contacts for further support as appropriate.
Main responsibilities will include:
Provide on a one to one basis (by telephone or personal visit as appropriate) accurate and sensitive information to people suffering from specific neurological conditions, their families and carers and day to day issues of living with the condition.
Provide accurate information on and/or signpost to relevant specialist local agencies providing information on welfare rights issues and appropriate services or support.
Maintain up to date information on resources in local statutory and voluntary services.
Signpost appropriate care pathways available locally to support sufferers at all stages of the condition (pre and diagnosis; maintenance and palliative)
Provide emotional support through regular contact when required and signpost to local charity support (branches and groups) and the national advisory service.
Provide up to date and relevant information and advice on welfare benefits that are particularly relevant to those living with neurological conditions (currently including Attendance Allowance; Disability Living Allowance and Carers); employment issues; home and residential care; mobility; local authority grant funding and other signposting as appropriate.
The successful candidate should have the following skills and experience:
Experience of providing one to one information to people in a health and/or social care setting
Experience of working as an advocate (either in a professional or personal life)
An awareness of local health and social care pathways
A good knowledge of appropriate support networks.
Knowledge of welfare benefits and/or knowledge of where to access welfare benefits advice.
Ideally, interested candidates should live in one of the local areas: Haringey, Hackney, Tower Hamlets, Islington and the City of London
It would be desirable for candidates to hold an existing, recent CRB check.
