We are looking for a Support Worker to join a well-established team for a leading support provider based in West London. The team supports young people with generic needs and we are looking for someone with outstanding knowledge in this area and the ability to develop person-centred relationships enabling clients to gain increasing independence and to develop their full potential.

Responsibilities will include:
• Working with vulnerable young people with varying needs
• Carrying out risk and need assessments, creating and implementing support plans depending on the individual s needs
• Liaising with external agencies and building relationships with service providers in the area
• Ensuring that policies and procedures are fully complied with

Candidates applying for this position MUST have:
• Passion for helping vulnerable people in the community
• The ability to liaise with external agencies
• Comprehensive knowledge of Supporting People funding and budgets
• An Enhanced CRB disclosure issued within the last 12 months
